This office is responsible for the issuance of certified birth, death, marriage certificates civil union and domestic partnership certificates.

The following is a printable form, which you can complete, print and mail with your payment. (You may also come into the Vital Statistics office to request copies.)

Marriage License/Civil Union Information

Applications are taken by appointment only. Office hours are Monday thru Friday 9:00 am to 4:30pm, with the last appointment of the day at 4:00pm. Couples may make an appointment for the application up to six months in advance but cannot pick up the license any earlier than 30 days prior to the wedding. The State requires a 72-hour waiting period from date of application to date of issuance.

Applicants should follow these rules when figuring out where to apply for a civil union/marriage license:

If the female/male applicant is a resident of New Jersey, the application is to be made in the municipality where he/she resides.
If the female is not a resident of New Jersey but the male is, the application is to be made in the municipality where he resides.
If neither are residents of New Jersey, the application is to be taken in the municipality where the marriage is to be performed.

The cost of a Marriage/Civil Union License is $28.00, payable when you pick up the license.

When applying for a license, the following information is needed:

Bride, Groom and 1 witness (18 yrs. of age or older). There is no minimum time period from which the witness knows the applicants.
Proof of residency of the bride (Driver's License, or bill that comes in the bride's name)
Social Security Cards or a paycheck showing SS#
Name, address and phone number of person performing the ceremony
Township in which ceremony will take place
Divorce papers, final judgment (with seal) or death certificate of prior spouse(s) where applicable

There is no longer a requirement for a premarital blood test; it was waived by the State of NJ.

There is a 72-hour waiting period from the time the application is signed until the license can be issued.

Domestic Partnership Certificates

Requirements for obtaining a Domestic Partnership Certificate in the Township of Edgewater Park.

Applicants do not need to apply in the town in which they live.

The application fee is $28.00.

Applications are taken by appointment only. Office hours are Monday thru Friday, 9:00 am to 4:30 pm, with the last appointment of the day at 4:00pm

Applicants must execute an Affidavit of Domestic Partnership in the presence of a Notary Public

File the notarized Affidavit of Domestic Partnership form with the Local Registrar of Vital Statistics in any municipality in New Jersey to obtain a Certificate of Domestic Partnership

Applicants must bring with them the following:

Current drivers license or photo ID with current address.
Provide proof of joint financial responsibility as evidenced by one of the following:
Joint deed, mortgage agreement or lease
Joint bank account
Designation of one of the persons as primary beneficiary on the other person's will:
Designation of one of the persons as primary beneficiary in the other person's life insurance policy or retirement plan; or
Joint ownership of a motor vehicle
